Community Care of West Virginia named 2024 Million Hearts Hypertension Control Champion

Community Care of West Virginia (CCWV) has achieved a monumental milestone by being named a 2024 Million Hearts Hypertension Control Champion. This prestigious recognition highlights CCWV’s outstanding commitment to improving cardiovascular health by prioritizing hypertension control among its patients. Notably, CCWV is only the second organization in West Virginia to receive this national honor.

Million Hearts, a national initiative co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS), honors organizations that have helped 80% or more of their patients with hypertension achieve blood pressure control. This achievement reflects the dedication of CCWV’s team in delivering lifesaving care and reducing the risk of heart disease and stroke in West Virginia communities.

“If all health care organizations nationwide could match this level of performance, we would prevent hundreds of thousands of cardiovascular events and save countless lives,” said Trish Collett, CEO of Community Care of West Virginia. “We are proud to be recognized for our efforts to ensure our patients live healthier, longer lives.”

Hypertension, or high blood pressure, affects nearly half of U.S. adults, yet many are unaware of their condition. Through innovative approaches, personalized care plans and a steadfast commitment to community health, CCWV has made significant strides in combating this silent epidemic.

As a 2024 Million Hearts Champion, CCWV is setting the standard for high-quality, patient-centered care and continues to lead the way in addressing one of the nation’s most pressing health challenges.
