Community Care of West Virginia (CCWV) has achieved a monumental milestone by being named a 2024 Million Hearts Hypertension Control Champion. This prestigious recognition highlights CCWV’s outstanding commitment to improving cardiovascular health by prioritizing hypertension control among its patients. Notably, CCWV is only the second organization in West Virginia to receive this national honor.
Million Hearts, a national initiative co-led by the Centers for Disease Control and Prevention (CDC) and the Centers for Medicare & Medicaid Services (CMS), honors organizations that have helped 80% or more of their patients with hypertension achieve blood pressure control. This achievement reflects the dedication of CCWV’s team in delivering lifesaving care and reducing the risk of heart disease and stroke in West Virginia communities.

Hypertension, or high blood pressure, affects nearly half of U.S. adults, yet many are unaware of their condition. Through innovative approaches, personalized care plans and a steadfast commitment to community health, CCWV has made significant strides in combating this silent epidemic.
